Facilities for Treatment of Irreversible Renal Disease


“Facility for the treatment of irreversible renal disease” means a facility that is not part of a hospital and which provides peritoneal dialysis or hemodialysis or trains a person with a permanent irreversible renal impairment to perform dialysis for himself or herself.

      To apply for CMS certification as a facility for the treatment of irreversible renal disease, you must provide the following:
