Many of the questions or concerns
the Patient Registry Cardholder Program gets are status checks on packet
requests or applications.
Currently, it takes about one
week 3 business days for the Program to send out an application packet from the
time the request arrives at the Division, and the application process
requires about 2 to 3 business days.
Due to privacy laws, we CANNOT
answer questions about applications over the telephone.
If you wish to check the status
of your application, you may do so via email, fax, or mail.

and include a copy of the front and back of your state issued ID to the
