Community Health Worker - Grants


The Community Health Worker (CHW) program does not operate under one funding source. The program receives an average of $300,000 per year to support activities. This program is 100% federally funded by various programs who are supported by the Centers for Disease Control and Prevention (CDC), the Substance Abuse and Mental Health Services Administration (SAMHSA), and the Health Resources and Services Administration (HRSA).
