Counseling, Testing, and Referral (CTR) Form

Counseling, Testing, and Referral (CTR) forms are designed as a data collection tool for EvaluationWeb®. This information helps CDC grantees who are collecting National HIV Prevention Program Monitoring and Evaluation (NHME) HIV testing data. The data collected on these forms provides a way to document to whom services are delivered and if the characteristics of the clients collected is similar to the ones that the agencies intended. Collecting this data also benefits agencies as state and Federal funding is often tied to the provision of services to specific populations. Also, data collected on CTR forms may illuminate emerging trends among communities that the agency was not expecting to serve or populations who may need interventions marketed toward them. Which, in turn, may inform efforts for the provision of other services in the community and/or referrals.

Rapid HIV Testing/Counseling Certification Program

This is a week long course designed to provide the attendee with all of the necessary information to conduct rapid HIV testing in our community. The breakdown of the course is below. In order to receive a certificate, you must attend all classes. This certificate is required by the State Laboratory Licensing Bureau for the CLIA waver to conduct on site Rapid HIV testing.
