The Cardiovascular Health Program

The goal of the Nevada Cardiovascular Health Program is to prevent and reduce premature death and disability from heart disease and stroke among Nevadans through:

  • Prevention, detection, and treatment of cardiovascular disease (CVD) risk factors.
  • Prompt and early detection and treatment of CVD.
  • Eliminating the disparities limiting health literacy and access to optimal cardiovascular health care.

    The Cardiovascular Health (CVH) program was established in 2013 and is 100% federally funded by the Centers for Disease Control and Prevention (CDC). The program receives an average of $1,661,814 annually to support infrastructure and health systems-based strategies for cardiovascular health. The CVH program focuses on modifiable risk factors targeting conditions such as high blood pressure or high blood cholesterol, social determinants of health such as education, income, neighborhood living conditions, and behavioral risk factors including tobacco use, diet, physical inactivity, and excessive alcohol use.

      The CVH program aligns activities around CDC-DP-23-0004, The National Cardiovascular Health program, and CDC-DP-23-0005, The Innovative Cardiovascular Health program to address the cardiovascular disease burden in Nevada. These activities are driven by partnerships with providers, patients, communities, and other stakeholders to implement both evidence-based and innovative strategies that have a measurable impact across the continuum of prevention to care, such as the heart-healthy ambassador-blood-pressure self-monitoring program (HHA-BPSM), statewide cardiovascular health learning collaboratives (LCs), or the fruit and veggie prescription (FVRx) program and encouraging healthier lifestyles and connections to supportive communities.

        Objectives

        • Build infrastructure by strengthening and promoting partnerships with state and local agencies, Nevada System of Higher Education (NSHE) employer groups, local and regional healthcare systems, and community-based organizations (CBOs).
        • Identify system-level gaps and disparities to address the burden of cardiovascular disease.
        • Increase the state's quality improvement standards of cardiac and stroke care through evidence-based guidelines.
        • Implement and promote policy and systems change interventions to address Nevada's heart disease and stroke burden.
