Here are some provider resources to help implement best practice protocols within your organization to reduce heart disease:
Hypertension Control Change Package 2nd Edition
The Hypertension Control Change Package for Clinicians (HCCP) presents a listing of process improvements that ambulatory clinical settings can implement as they seek optimal hypertension (HTN) control. It is composed of change concepts, change ideas, evidence- or practice based tools and resources.
High Blood Pressure Resource Toolkit
High blood pressure currently affects about 70 million Americans, with an additional 1 in 5 adults unaware they have high blood pressure. High blood pressure increases risks of heart attack, stroke, heart failure, kidney disease, and other chronic diseases. Knowledge and lifestyle modifications are important to maintaining a healthy blood pressure. This provider resource includes detailed information pertaining to clinical guidelines and recommendations for monitoring blood pressure, self-monitoring interventions, and information fact sheets for dissemination to patients.
Stroke Resource Toolkits
Stroke is the fifth-leading cause of death in Nevada and also a leading cause of serious long-term disability. Strokes can be prevented with education about modifiable risk factors, awareness of signs and symptoms, early interventions and access to care. This “Stroke Resource Toolkit” offers detailed information for providers including the NIH acute stroke assessment, treatment resources, and informational sheets to complement conversations with patients and families.
American Heart Association's Target BP Program
The AHA and AMA partnered to launch Target: BP in 2015 to improve blood pressure control and build a healthier nation. This national initiative aims to reduce the number of Americans who have heart attacks and strokes by urging medical practices, health service organizations, and patients to prioritize blood pressure control.
Based on the most current AHA guidelines, Target: BP supports physicians and care teams by offering access to the latest research, tools, and resources to reach and sustain blood pressure goal rates of less than 140/90 mmHg within the patients populations they serve.
Working together, medical practices and health service organizations can significantly improve the nation’s current national blood pressure control rate of 54 percent. Join Target: BP and be a part of this national movement to build a healthier America, starting today.
Cardiac Rehabilitation Change Package
This Cardiac Rehabilitation Change Package was completed by the Centers for Disease Control and Prevention (CDC) in collaboration with the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR) with the purpose of helping cardiac rehabilitation programs, hospital quality improvement teams, and public health professionals who partner with these groups to implement systems and strategies that improve care for patients who are eligible for cardiac rehabilitation.
