Health Information Services Mission

The mission of the HIS Department is to provide a comprehensive inpatient and outpatient medical record system which meets licensure and accreditation standards and promotes the ultimate mission of providing quality clinical services, care and treatment to our clients.

Request Medical Records

If you need a copy of your medical records from a facility operated by the Division of Public and Behavioral Health, please submit your medical records request directly to the facility. Contact information for each facility is available here
